Client Questions in the event of an address change:
1. List your New Address and attach a legal document which validates this address. (For example W9, business license, government notification, etc.)
3. Did most or all of the employees move with you?
4. How did this move impact your Quality Management System (QMS)?
5. Has your Quality Manual been updated since this move?
6. Is your employee count the same?
7. Is your number of shifts the same?
8. Are there any changes to support sites or support functions (if any)?
9. When do you expect to be in your new facility?
10. Does this address change affect your billing?
11. Is there any production equipment being moved that requires validation and if so, when will that validation be completed?
