Services provided at discharge or transition of child to another facility.
The transitioning of children from one placement to the next is a very important process for Thrive.
We work very hard with the child’s caseworker and the next placement in making all transition as
smooth as possible.
1. Open communication between placements is critical.
2. Transition process individualized for each child: We will work with the case manager to assist in all
we can.
3. All proper documentation (placement packet, etc.) will follow child to next placement.
4. Working closely with case manager to ensure services are in place before child is moved:
Psychology evaluations, Med reviews, placement packet completed.
5. Having services in place in a timely manner ensure that the child and the next placement have a
successful transition.
6. On site visitation of transitional placement as approved by the case manager.
7. Services provided to family when appropriate and approved by case manager: Food and Clothing
assistance, parenting classes, etc.
All transition will have impact upon the child. Whether that impact is good or bad is depending
on timely communication with the child and allowing the child time to process the transition.
Attachments are always made at any placement and allowing closure for a child is one of the
most important aspect of our discharge planning.
