Attached is the enclosed letter regarding a potential safety issue involving the Avance, Avance CS2 and Aisys anesthesia machines.
Please ensure that all potential users in your facility are made aware of this safety notification and the recommended actions.
BACKGROUND:
A potential tidal volume over delivery when using Pressure Control Ventilation – Volume Guarantee (PCV-VG) ventilation mode when given a unique sequence of user inputs and a collapsed bellows (partially filled circuit).
INSTRUCTIONS in this letter – For User:
The user can continue to use the anesthesia system as well as the PCV-VG mode, but need to take the following actions/ attentions:
• To avoid this situation, do not adjust the ventilator tidal volume while in a manual ventilation mode.
• At any time should an over delivery of tidal volume occur while in this ventilation mode, transitioning to a different ventilation mode will resolve the issue.
INSTRUCTIONS – Your attention:
1) Please review the affected units in the excel sheet.
We need your inputs for end customers location (in column “G” and “H”) so that we can submit to factory our IB validation on this process.
Without it, there could be a delay of the FMI kits set up for your country.
