EARS:
Ear both symmetric. No secretions or inflammation of the ear. Both ears hear clearly. Lymph nodes in ear
NOSE :
Both nose is not distorted. No nasal secretions out of the sinuses symmetric swelling cannot hurt no odor
MOUTH:
Lips have pale sligtiy and no cyanosis , no stomatitis , tongue not deviated , have dental caries or gingivitis, pharynx not injected , uvula midline, tonsil not enlarged, soft and hard palate no lesion.No cavities
NECK:
Trachea in midline , jugular vein not engorged , thyroid grand not enlarged , lymph node not palpable movement no limitation, no stiffness of neck .
CHEST, LUNG :
Symmetrical chest wall ,chest normal shape and size and movement , lung clear, lung expand both lung no retraction, normal breathing sound both lung , no adventitious sound.
HEART:
Pulse 92 / min regular, Heart rate 20 /min regular, no murmur.
ABDOMEN:
Tympanic percussion sounds/Long low-pitch sounds, no mass , no guarding , liver and spleen can’t be palpated . Bowel sound 3 / min ,no incision
MUSCULOSKELETAL:
Right armleft , leg and left arm normal movement butfracture right leg, no paresthesia , have stump of right ,no paralysis , Left arm and left motor power grade 5 , Right arm motor power grade 5, Right leg motor power grade 3
NEUROLOGICAL SIGN:
Glasgow Coma score = 15 (E4V5M6)
