Clinical Implications
Our study explored associations among selected demographics, childbirth-related stressors, depressive symptoms, gestational age, and PBT and determined group differences among adolescents giving birth prior to 38 weeks and 38 to 42 weeks gestation. Adolescents giving birth before 38 weeks more frequently experienced cesarean births, which may suggest a common standard of care for early gestational age births or other unknown, concurrent obstetrical complications. Adolescents are a vulnerable population for an early birth and the psychological consequences that may follow. A history of a preterm birth is a significant risk factor for a future preterm birth; therefore, nurses should provide educational information to adolescents related to repeat pregnancy, risk factors for preterm births, and PBT.
The majority of study adolescents reported kind and supportive caregivers for labor and delivery; however, adolescents giving birth before 38 weeks reported less support. Nonsupportive caregivers can contribute to negative birth experiences with long-term negative birth memories (Stadlmayr et al., 2006)
Perception of caregiver support, or kindness, may be evaluated in part by the adolescent's perception of pain control. Although most adolescents reported adequate pain control adolescents giving birth before 38 weeks expressed more concern with inadequate management. Linked to a negative birth appraisal, adolescents who define their birth experience by pain, will define their birth experience as negative (Low, Martin, Sampselle, Guthrie, Oakley, 2003.) Because adolescents with unplanned pregnancies may be less prepared for birth, especially a cesarean, education directed at labor processes and pain expectations, including methods of pain control, may help reduce symptoms of PBT.
In the current study 13% to 24% of adolescents from both groups failed to complete the CES-D for depressive symptoms, which may have prevented finding a correlation between depressive symptoms and gestational age, an association reported elsewhere in the literature (Dole et al., 2003). However, despite missing data our study did find a correlation between depressive symptoms and IES scores, supporting the recognized comorbidity of depression and PTSD/posttraumatic stress (White et al., 2006). Because of the high number of adolescents known to suffer with depressive symptoms, perinatal screening for depressive symptoms and a careful watch for signs of PBT (psychological birth trauma) is encouraged. Perinatal assessments should routinely include information related to past history or current depression and previous birth trauma, when applicable for multiparous adolescents. Hospitals are encouraged to consider a motherbaby care system, which can arrange and coordinate postpartum follow-up, especially for adolescents vulnerable to or symptomatic of depression and PBT (psychological birth trauma) at the time of birth. This system would provide a means of determining if symptoms signal a transient, acute stress reaction because of an overwhelming life event or long-term psychological consequences. Limitations of the study including lack of power analysis, small sample size with bias, and missing data may have prevented finding associations previously identified in the literature. A second limitation relates to gestational age. To most healthcare professionals, preterm is defined as birth under 37 weeks gestation; however, for this study an older definition was used defining the early birth as under 38 weeks gestation. It is unknown how many adolescents gave birth at 37 weeks and were mislabeled as early rather than full-term births Measurement and design weaknesses include the use of screening rather than diagnostic i which may over or underestimate depressive symptoms or actual trauma impact following child- birth. A one item rating measure for birth appraisal may not have captured the true birth stress experienced by the adolescent with arbitrarily set points of classification; however, some important information was yielded with its use. Research exploring PBT among multi- ethnic-racial adolescents is rare in the literature. What exists is information describing primarily adult, non-American women. How American adolescents react to the experience of birth and what pre- pregnancy, prenatal, or postpartum vari- ables influence their reactions is rela tively unknown. Correlations not identi- fied in this study may be forthcoming through additional research among larg- er, more diverse groups of adolescents.
