Do not use a mechanical lift for a patient with a recent total hip replacement or a patient who is at risk for hip dislocation.
Position other personnel at the head of the patient's bed to protect and support the patient's head and neck if he or she is weak or unable to assist with a lift transfer.
Perform hand hygiene.Verify the correct patient using two identifiers.Assess the patient's physiologic capability fortransfer. Compare findings to baseline assessment and report any decreased ability to the primary care practitioner and the rehabilitation staff. Restrict individual lifting to a maximum of 35 lb (15.9 kg) of the patient's weight.Do not start the procedure until all required personnel are available.Perform hand hygiene and don gloves.Bring the lift to the bedside or lower the ceiling lift and position properly.Position a chair near the bed in a manner that reduces the distance the lift will have to travel while also allowing adequate space to maneuver the lift. If using a wheelchair, lock the chair brakes, remove the footrests, and lock the bed brakes.Standing on one side of the bed, raise the bed to the high position with the mattress flat. Lower the side rail.Raise the opposite side rail for patient safety unless a second nurse is assisting.Roll the patient onto his or her side facing the raised bed rail or assisting personnel.Place the hammock sling or canvas strips under the patient in the proper location. If using canvas strips (two pieces), place the lower edge (wide piece) under the patient's knees and place the upper edge (narrow piece) under the patient's shoulders. A u-type sling should be placed such that the cutout is at the base of the spine.Place the hammock sling under the patient's center of gravity and the greatest portion of body weight. Ensure that the hooks face away from the patient's skin.Raise the bed rail on the side closest to the chair.Move to the opposite side of the bed and lower the near side rail.Roll the patient facing the opposite side and pull the hammock sling (or canvas strips) through, and unfold and smooth the hammock sling over the bed surface.Roll the patient supine onto the hammock sling. Ensure that the sling extends from shoulders to knees to support the patient's body weight equally.Remove the patient's glasses, if appropriate.Place the lift's horseshoe bar under the side of the bed (on the side with the chair).Lower the horizontal bar to sling level by following the manufacturer's instructions. Lock the check valve if required for the lift that is used.Attach hooks on the strap or chain to the loops or grommets in the sling. Short chains or straps hook to top loops or grommets in the sling; longer chains hook to the bottom of the sling.Elevate the head of the bed, positioning the patient in a sitting position.Fold the patient's arms over the chest (Figure 4Pump the hydraulic handle using long, slow, even strokes until the patient is raised off the bed (Figure 4). For a ceiling lift, turn on the control device to move the lift.Use the steering handle to pull the lift from the bed and maneuver the patient from the bed to the chair.Roll the base around the chair, positioning the lift in front of the chair.Release the check valve slowly (turn to the left) and lower the patient into the chair (Figure 1). For a ceiling lift, use the control device to lower the patient.Close the check valve or turn off the control device as soon as the patient is completely in the chair seat and the straps can be released.Remove straps and mechanical or hydraulic lift.Ensure the patient's proper alignment in the sitting position: Head is erect and vertebrae are in straight alignment. Body weight is evenly distributed on buttocks and thighs. Thighs are parallel and in horizontal plane. Both feet are supported on the floor, and ankles are comfortably flexed. An adequate space is maintained between the edge of the seat and the popliteal space on the posterior surface of the knee.Remain in front of the patient until the patient regains balance and continue to provide physical support if the patient is weak or cognitively impaired.Provide support for any paralyzed extremities. If the patient has a flaccid arm, support it with a lap board or sling; if the patient has a flaccid leg, stabilize the leg with a bath blanket or pillow.Monitor vital signs. Ask if the patient feels dizzy or fatigued.Note the patient's behavioral response to thetransfer.Assess, treat, and reassess pain.Remove gloves and perform hand hygiene.Document the procedure in the patien's record
