Extracellular fluid volume increases by up to 30% inpatients with advanced CKD and severe edema. Loop diureticsare the recommended diuretics for patients whohave an estimated glomerular filtration rate (eGFR)<30mL/min/1.73 m2 because other diuretics, including thiazide,are less effective in advanced CKD15). The natriureticresponse to furosemide is also reduced in patients withCKD4). This is because Na+ reabsorption in the downstreamsegment is increased, and the renal delivery of furosemideis reduced according to the decrease in the GFR.Compared with the general population, only about 15-20% of the furosemide dose is delivered into the tubularfluid in stage 5 CKD patients1). This diminished tubularsecretion is due to the elevated level of endogenous organicanions that interfere with furosemide secretion via organicacid transporters in the proximal tubule4,16). Therefore,to achieve the desired effect, higher doses or an increasedfrequency of furosemide treatment is required toincrease tubular secretion.
