While studies have supported the association between effective leadership and
employee outcomes, it may be that leadership can positively influence outcomes for
patients in healthcare organizations as well. In a correlational study conducted by
McNeese-Smith (1999), significant relationships were found between managerial
motivation for achievement, the use of Kouzes and Posner's (1995) leadership practices,
job satisfaction, productivity, organizational commitment, and patient satisfaction.
Although this study was examining nurse manager motivation in relation to leadership
practices and outcome variables, the findings were consistent with earlier studies
demonstrating significant positive relationships between managers' use of Kouzes and
Posner's (1995) five leadership practices and organizational outcomes (McNeese, 1995).
In addition, managers' use of the five leadership practices have been positively linked to
higher levels of staff nurse expertise and staff stability reflected in the retention of staff
nurses (Houser, 2003). In Houser's study, a model of contextual factors within the
nursing work environment influencing patient outcomes was tested. Although the sample
size was small, high scores of leadership practices related to higher levels of staff nurse
expertise which were associated with a significant decrease in the incidence of adverse
patient events (Houser, 2003). These relationships are pertinent to this study as staff
nurse clinical expertise has been recognized in the literature as an important attribute of
clinical leadership. While these findings support an indirect link between the use of
Kouzes and Posner's (1995) five leadership practices and positive outcomes for patients,
further research is needed to better understand the relationships between effective
leadership and patient outcomes at both the managerial and staff nurse level.
