While loading NGL from NGL truck into NGL Tank at Akibam Oil Storage, the loading motor irregularly produced high heat and initiated small sparks. The sparks caused fire to the electrical wire around the area. Staffs at loading station spotted the sparks and quickly shutdown breaker at electrical control box to shut down all electrical systems. A staff who was behind the loading truck closed the emergency valve at the back of the truck, he saw the fire breaking out so he shouted out to inform everyone. While jumping to help his friends, he stumbled over the drain and broke his ankle. ECB’s driver was found injured and unconscious at the back of the truck near the scene of the fire. HEAD ERT (On Scene Commander) has conducted a head count and confirmed 1 Akibam staff injured and 1 ECB driver injured and unconscious. He Head ERT radioed for help and informed Adami’s Emergency Management Team (EMT) and MILS EMT on this incident in Akibam Oil tank Storage. HEAD ERT /On Scene Commander (OSC) to assess the fire and report back to the Head ERT. ERT was activated.
The receiving and loading staffs who witnessed the incident helped to stop the fire by using dry chemical fire extinguisher to contain the fire (real live simulation and exercise), but were unable to control the fire outbreak so they contact the head of the operation to inform send report of the incident to head of emergency response team and to set on the emergency alarm. The report said a fire broke out while receiving NGL from NGL truck and some staffs are injured from broken ankle so aid team and sick bed is needed to rescue the injured from incident area. ERT started Akibam’s main fire fighting system and commenced boundary cooling of all oil Storage tanks.
Head of emergency response team (ERT) contact the communication team to alert everyone on the incident and start the evacuation of everyone to assembly point and perform head count to check if everyone was there. Adami’s Emergency Management Team was informed to be on standby. Head of emergency response team evaluate the incident and think it is out of control and needs external support so he ordered the communication department to contact the nearest firefighting unit to Khao Roop Chang municipal for support and contact the ambulance and aiding nurse to evacuate the injured. When the municipal’s firefighting unit arrived and join hands with Akibam Oil’s emergency response team, fire was put out. Authorized personnel get inside the affected area to examine the situation. They found that fire was completely put out and will not reignite so the emergency response was completed and responsible personnel on restoration will jump in to kick start their part.
HEAD ERT (On Scene Commander/OSC) after confirming that the Emergency in Akibam is under control, fire extinguished, the injured and unconscious are taken to the hospital, the Head ERT declared standown.
