A cardiology consult was requested on postoperative day 8 as clinical symptoms of SOB, adventitious breath sounds, and hypertension continued. A beta-blocker and an ACE inhibitor were initiated and titrated according to BP trend. An echocardiogram was preformed and LIEF measured 35% suggesting PPCM with moderate left ventricular dysfunction. Diuretic therapy was continued and the woman was transferred to the progressive care telemetry unit where BNP levels were drown and found to be severely elevated at 1847.0 pg/mL. Upon arrival to the unit the patient was alert, but reported being very tired. She was anxious about seeing her babies in the NICU , therefore, a wheelchair was provided and she was transported to the NICU by her family. Due to the cardiac medication, it was recommended by the LC that expressed and not given to the babies, however, she should continue to pump to maintain her milk supply.
