Stage II NEC is considered definite NEC and includes nondescript symptoms plus severe abdominal distention and tenderness, grossly bloody stools, and the radiologic findings of pneumatosis intestinalis or portal venous gas. Pneumatosis intestinalis is the presence of submucosal or subserosal air in the intestines, while portal venous gas is present when the pneumatosis intestinalis extends into the portal circulation.
