The investigator prepares a written report. Sections of the report may include:
* Executive summary
* Table of contents
* Background information about the service
* Reasons why the survey was undertaken
* Narrative description of the results of each question
* Graphic representation of the results of each question
* (See Appendix 3 for an example)
* Narrative description and graphic representation of data combined from several questions
* Where appropriate, present internal statistics that support or refute the survey's findings
* Compare current results with previous results if a similar survey was administered in the past
* Conclusion and recommendations
