Proceed with creating an account using an email address and password. When filling in the required fields pertaining to the account, the end user will have to input the Principal Investigator (PI), Institutional Official (IO), mailing, and the shipping & billing information. After account set up, an activation email should arrive at your email address. You will then click the link to activate your account.
After account set up and approval, you will then be able to place an order by placing the items of interest into your shopping cart.
In regards to payment, during the ordering process, when the customer arrives at the "payment" step, this is where you will input the purchase order or credit card number that you have to pay for the materials. If paying by check or bank transfer, please note that. When you are at the payment step, you may also enter your FedEx account number if you would like to be billed directly for shipping. Otherwise, shipping will be invoiced to you on your receipt once the order has shipped
The statement of research intent for the order will be typed into a free text box during the ordering process. When you reach the "secondary distribution" step, please note if you intend to share or collaborate with other investigators using the acquired materials. If there will be any sharing/collaboration, you will need to provide Coriell with a material transfer agreement for any collaborating scientists. If there will be no sharing of the materials outside of the PI's group, then you will choose the "Used in my laboratory under my direct supervision" option when you reach this step.
All materials require material transfer agreements as well. The customer will find these online under the appropriate collection from which they are requesting the samples. You will also be notified during the checkout process of the Materials Transfer Agreement (MTA) needed for the order. The PI and IO associated with the account should be the same PI and IO who will sign the Materials Transfer Agreement. The PI and IO should be two separate individuals ideally. Once you have completed the checkout process, you will receive a confirmation "Pre-order" number referencing the order.
