BILATERAL MAMMOGRAPHY WITH ULTRASOUND OF THE BREASTS



CLINICAL INFORMATION




Screening mammogram



TECHNIQUE




Digital mammogram in standard view.





FINDINGS




Mammographic findings:

Breast composition: Heterogeneously dense breast tissue, which could obscured detection of small masses.

No discrete mass or architectural distortion or suspicious malignant microcalcification.

Unchanged some nodules in both breasts, corresponding with cysts from US.

Unchanged a benign popcorn macrocalcification at mid quadrant left breast, suggestive of calcified

involuting fibroadenoma.

No change of a 5 mm, nodule in UOQ left breast, probable benign intramammary node.



Sonographic findings:

Few prominent bilateral subareolar ducts, favor benign ductal ectasia.

Some thin wall cysts in both breasts, size up to 5.6 mm

Two complicated cysts at 12 Hr left breast, size 3.2x5.9 mm and 3.1x5.5 mm

A 4.6 mm, calcification at medial aspect of left periareolar area.

No skin thickening or parenchymal distortion.



IMPRESSION




1. A benign calcified involuting fibroadenoma in left breast.

2. Some bilateral benign breast cysts.

3. Two complicated cysts in left breast, favor benign.

4. Few benign ductal ectasia in both breasts.

5. Unchanged a nodule in left breast, probable benign intramammary node.

No mammographic evidence of malignancy.

No significant change of findings from the study on 11 Oct 2010 and 13 Mar 2008.



ASSESSMENT AND RECOMMENDATION




BI-RADS

2 [Benign findings], advise annual mammogram and sonogram.



Note:

- Average false negative rate of mammography is 10-15 %.
- Management of a palpable abnormality must be based on clinical consideration.
- Adenosis and dense breast parenchyma may obscure an underlying neoplasm

พบปัญหาการวิจัยส่วนประกอบของเต้านม : โอดโอยหนาทึบเนื้อเยื่อเต้านม ซึ่งอาจบดบังการตรวจหาฝูงเล็ก ๆ

ไม่มีมวลไม่ต่อเนื่องหรือบิดเบือน สถาปัตยกรรม หรือสงสัยมะเร็ง microcalcification

ไม่เปลี่ยนแปลงบางก้อนในเต้านมทั้งสองสอดคล้องกับเชื้อจากเรา

ไม่เปลี่ยนแปลงเป็นเนื้องอกที่กลางอกซ้ายของข้าวโพดคั่ว macrocalcification , ส่วนชี้นํา

involuting ไฟโบรอะดีโนมา

ไม่มีการเปลี่ยนแปลงของ 5 มิลลิเมตร ปมใน uoq ซ้ายน่าจะเป็นเนื้องอกเต้านม intramammary ปม



sonographic ผลการวิจัย :

ไม่กี่ที่โดดเด่นทวิภาคี subareolar ท่ออย่างอ่อนโยน ductal อาจาร

บางผนังซีสต์ในเต้านมทั้งสองบาง ขนาดถึง 5.6 มิลลิเมตร

สองซับซ้อนซีสต์ที่เต้านมซ้าย 12 ชม. , อืม 3.2x5.9 ขนาดและ 3.1x5.5 มม

4.6 มิลลิเมตร ด้าน medial ของหินปูนที่ periareolar

เหลือพื้นที่ไม่มีความหนาผิวหรือ parenchymal เพี้ยน



ประทับใจ




1 อ่อนโยน ส่วน involuting ไฟโบรอะดีโนมาในเต้านมซ้าย

2 บางก้อนซีสต์เต้านมทั้งสอง

3 สองซับซ้อนซีสต์ในเต้านมซ้ายอย่างอ่อนโยน

4 . อาจารน้อย ใจดี ductal ในเต้านมทั้งสอง

5 ไม่เปลี่ยนแปลงเป็นปมในซ้ายน่าจะเป็นเนื้องอกเต้านม intramammary ปม

ไม่มีหลักฐานปัญหาของเนื้อร้าย

ไม่พบการเปลี่ยนแปลงของผลการวิจัยจากการศึกษาในวันที่ 11 ต.ค. และ 13 มี.ค. 2008 .
