Dear Customers,
The American Heart Association (AHA) and the European Resuscitation Council (ERC) 2015 Guidelines for Resuscitation and CPR (2015 Guidelines) were released online on October 15, 2015. These 2015 Guidelines are the culmination of the work done to develop science-based recommendations for treating cardiovascular emergencies, particularly sudden cardiac arrest.
Physio-Control recognizes these Guidelines have the potential to positively impact patient outcomes. The day the new Guidelines were released, Physio-Control convened an expert working group to assess and summarize the key changes.
The 2015 Guidelines contain more detail than we can include in this update. However, I wanted to share some initial observations of the content:
Strong recommendation for using social media apps and cell phones to speed bystander CPR for cardiac arrest victims.
Quality improvement of CPR performance continues to be a major emphasis in 2015.
An upper rate limit for chest compressions of 120 and recommended range of 100-120 compressions/minute and an upper chest compression depth of 6 cm (2.4 inches) and recommended range of 5-6cm (2-2.4 inches).
Mechanical chest compression devices may be considered as a reasonable alternative when sustained high-quality manual CPR is challenging or dangerous for the provider (e.g., prolonged CPR, transport, cath lab).
Systems of Care and Continuous Quality Improvement now define a separate Chain of Survival for hospital and for prehospital. OHCA depends on community for support, while IHCA depends on a system of surveillance.
For CPR feedback technology, "it may be reasonable to use audiovisual feedback devices during CPR for real-time optimization of CPR performance". Although, "the use of CPR feedback devices has not been shown to significantly improve performance of chest compression depth, chest compression fraction, and ventilation rate."
The routine use of the ITD (impedance threshold device) and the ACD-CPR and ITD during CPR is not recommended as an alternative to conventional CPR.
Several strategies are emphasized for the treatment of possible Acute Coronary Syndrome patients, including acquisition of a prehospital 12-lead ECG and prehospital activation of the catheterization laboratory for all patients with a recognized STEMI. In addition, trained non-physicians may interpret 12-lead ECGs for STEMI and computerized 12-lead ECG interpretation should not be used as the sole means to recognize STEMI.
Continuous waveform capnography is recommended, in addition to clinical assessment, as the most reliable method of confirming and monitoring correct placement of an ETT.
Regarding biphasic energy defibrillation, there were minimal changes in the Guidelines. It is reasonable to use the manufacturer's recommended energy dose for the first shock. If this is not known, defibrillation at the maximal dose may be considered. In addition, if using a manual defibrillator capable of escalating energies, higher energies for a second and subsequent shocks may be considered.
