2) Ciggarate buds observed at site near utility building, fabrication yard. This is very serious observation
3) Housekeeping needs to be improved in all areas. This is MUST PRIORITY action
4) All scaffolds to be thoroughly inspected and provide toe guards to protect potential drop zones. After inspection scaffolds need to be properly tagged
5) All deep excavated areas in the Production building need to be properly hard barricaded and appropriate caution signages to be displayed
6) Oxygen and Acetylene gas cylinders should not be kept on one trolley. As per BASF requirements, there has to be minimum 5 M distance between two cylinders. This requirement has been informed last week but not yet implemented. Also the cylinders should be labelled.
7) Unwanted shuttering material beside Fabrication yard need to be removed or to be arranged properly
8) All hand rails/mid rails to be properly aligned
9) Signages such as “Way to Assembly Area”, “Safety harness for Height work”, “Emergency Escape Routes with location mentioning “You are Here”, “Deep Excavation Area” are missing in many areas. Please provide the signages on priority
10) Denailing of all deshuttered wooden planks to be carried out on PRIORITY. This action is pending since many weeks
11) All equipments at site need to be inspected and ensure that monthly code sticker fro May is displayed. Also ensure inspection checklists for all equipments need to be kept ready. All fire extinguishers should have inspection stickers.
12) Unwanted PPEs should not be seen at the workplace area.
13) All electrical cables should be routed overhead. Use of insulated “S” hooks is mandatory. Do not take support of scaffold for laying cables
14) Ensure proper access to all work areas. No trip/slip hazards should be observed in the access ways.
15) Caution signage for fire fought tank is mandatory ( Drowning Risk, No Drinking Water). Restrict access to the tank area.
We expect these points to be closed by the end of day without any further delay and reminders. We shall again have a site visit at 2.30pm today to see the progress in closing the above points. Is there is no progress seen in taken corrective actions we may have to stop work at site to implement the above observations.
