• The community center’s case management should be classified as “Intensive”, “General” and “Simple”.
• Standard
1) Intensive: is complexity classified according to a case meeting with team members
2) General: is the case that receive at least 3 more services from the center
3) Simple: is the case that receive 1 or 2 services from the center
• Counseling(or home visit)
1) Intensive case: Visit twice per month
2) General case: Visit once per month
3) Simple case: Visit once per quarter
• Provide services to clients
• Provide services to each client by cooperation of other organization and network
• To follow up of each client’s life plan (home visit or meeting at GPL office, phone call etc.)
• Prepare follow up report in each case.
• Prepare ‘process record sheet’ with following (it could be simple record or detail record base on actual case) and suppose to attached each case file
• ‘Process record sheet’ on case management should be written and approved quarterly.
• Case meeting(Group supervision / Peer feedback) with team members to find possible solution for clients : once a month
• It could be discuss with director about urgent serious case
• Write minute after case meeting
• Sharing process and achievement of life plan by each client
• Sharing Action plan by each client
• Sharing good practices
• Special training based on client’s needs
• Quarterly meeting should be had at GPL quarterly
