The sample was drawn from two sites. The staff at site A, an advanced-practice
nurse-coordinated diabetes education center in a New Jersey suburb, sent a recruitment
letter to all the individuals ( N = 2,655) on a confidential patient mailing list (not
viewed by the investigator) used to send promotional materials about upcoming
health programs. Individuals were asked to return a prestamped, coded (for privacy)
postcard indicating their interest in participation. They were offered a $5.00 donation
to a diabetes charity, customary to this site. The response rate was 4.5% ( n = 138).
However, the mailing went to everyone who ever attended a diabetes education
program at the site, including those who would be ineligible, such as those with a
negative family history, type 1 diabetes, or gestational diabetes.
