Please create a Workers Allowance C

Please create a Workers Allowance Claim Form. This form should have the following:

1. Supervisor applying for allowance for workers
2. Date and duration of work
3. Address of worksite
4. Names of workers who are given the allowance
5. Rate of allowance per day
6. Type of allowance
7. Signature of supervisor receiving the total amount
8. Signature of worker to indicate receipt of allowance
9. Any other information

This form should be filled up to be given to Accounting Dept to request for petty cash each time the workers are assigned to work in a off-factory worksite.

Please have this form created by 13th Dec and send to Khun Kong and myself to approve the draft.

We intend to start using this form for the demo next week.
