In 2009, a large tertiary center joined the obstetric
hemorrhage multihospital collaborative organized
by the CMQCC. Prior to 2009, postpartum hemorrhage
was included on the critical events debriefing
template but was not reliably completed.
The new performance improvement process provided
more specific criteria on when and how to
debrief for a hemorrhage. Currently, the perinatal
unit debriefs on 80% to 100% of all postpartum
hemorrhage cases that meet the performance improvement
criteria. The CMQCC debriefing checklist
has been a useful tool to track the team’s
response during an obstetric hemorrhage emergency.
Lessons from these debriefings are shared
with the staff to identify good practices and outcomes
as well as areas for continued improvement.
