In the Navy if you are sick on ship out at sea it's just tough luck because they do not let you out of work unless you have no pulse or not breathing. A lot of doctors think you are faking it to get out of work so I had to try and keep working. My stomach and headaches were terrible and the ship's doctor just ignores me so finally I went to the Commander and they he let me use my own vacation time (that was wrong it should have been sick leave) so I can fly off ship and go back home and see a real doctor that can do lab tests. The headache was excruciating pain. The doctor at the hospital where I lived said I had a H Pylori infection in my stomach and treated me with some strong antibiotics for 2 weeks. Navy ships out at sea do not have any laboratories to do tests and they do nothing unless your vital signs are very bad. But the problem is that many illnesses do not manifest themselves with bad vital signs so they ignore you.
