Dear Medic,

PTTEP ongoing to set-up KPI for occupational health management, we designed the OH management report form to include the medical and occupational health activities in the field for monthly report. The report will be on the followings:

1) Leading indicators: all occupational health activities to prevent occupational illness
2) Lagging indicators: including Occupational Illness cases and numbers and cost of MEDEVAC, medical referral.


Please review detail as require data in the template/form and input data as require then send me by Feb. 6th 2015
