An MPT Final Assembly operator began to disassemble a Transformer. The angle bracket under the radiator section needed to be removed so the operator had loosened a bolt at one end and was in the process of unfastening the securing bolt at the other end when the bracket fell. The operators left hand was holding the bracket and as it pivoted on the secured bolt a scissor/shear action occurred resulting in the left thumb becoming caught between the angle bracket and radiator. The resulting injury entailed a broken finger and tearing of the tendon.
The key learning points from this incident are
1) Complete risk assessment prior to activity/job task taking place, especially for non-routine activities.
2) Ensure all hands are kept clear from potential trap/shear points
3) Follow strictly work instructions, particularly where 2 persons activities are required.
