What Do I Need to Tell the Patient/Patient’s Family?
› Educate the patient/family members about what to expect during and after receiving CC preventive cardiovascular care. Encourage questions
› Explain how to contact the treating clinician if questions or problems arise
› Provide written information, if available, to reinforce verbal education, especially if family members or other nonprofessional caregivers will participate in providing care at home
› Encourage adhering to the schedule for continued medical surveillance. Provide written instructions with dates and times of scheduled appointments, if available
› As appropriate, encourage attending a community support group for contact with other Black patients concerned about preventing CVD.
