ALERT
Decide whether to use an antiseptic soap based upon the procedure to be performed and the patient’s immune status.
Take care to avoid tearing or cutting skin under or around nails.
Perform hand hygiene before patient contact.
Verify the correct patient using two identifiers.
Inspect surface of hands for dermatitis or breaks or cuts in skin or cuticles. Avoid long or artificial nails.
Inspect hands for visible soiling.
Note condition of nails. Be sure fingernails are short, filed, and smooth.
Cover any skin lesions before providing patient care.
Push long uniform sleeves above wrists. Avoid wearing rings or a watch; if worn, remove during hand hygiene. If rings are replaced after hand hygiene, perform hand hygiene again.
