At approximately 9:45 a.m. on Thursday, March 24, 2016 at a remote platform, two (2) contractor personnel were attempting to remove a Back Pressure Valve (BPV) at well no. 22 with a dry rod type BPV retrieving tool without using a BPV lubricator. The crew attempted to release the pressure under the BPV by manually turning the dry rod about 3-4 counterclockwise direction. Suddenly the BPV and the attached dry rod released and ejected upward through an open hatch above the X-mas tree, striking the BOP ram and landing on the BOP deck. According to the BPV design specification, it requires approximately 5 turns counterclockwise to release the pressure and additional 9 turns counterclockwise to release the BPV from the tubing hanger. There were no injuries or equipment damage. There were six (6) people working on the BOP deck and two people located on the well head deck at the time of the event.
