Nurses
need to be aware that when patients leave the hospital with
little or no discharge information and/or information that
is inaccurate, they are more likely to develop concerns or
problems that require them to access health care agencies
following discharge. In addition, staff development educators may need to focus more on developing educational
programmes emphasizing discharge planning and nurse
educators need to ensure that discharge teaching and its
relationship to self-care is emphasized in undergraduate
nursing curricula.
