Clinicians commonly observe that some patients make associations between their
own development of diabetes or its related complications and those developed by
family members before them. A patient’s tendency to make social comparisons to
others with a disease similar to their own can influence their self-care behavior.
For example, some patients who see a similarity between themselves and a family
member seek to reinforce their experience of diabetes by further adapting their
perceptions and modeling their family member’s behavior. On the other hand, other
patients react against the family member in a self-enhancing way by forming different
perceptions and engaging in self-care behaviors that are healthier than those of their
family member. Some patients exert great effort to change what they believe would
be their own trajectory of diabetes based on their family history. Yet others report
embracing the self-fulfilling prophecy in a fatalistic way. In either case, the result may
be optimal, suboptimal, or inconsistent self-care behavior, depending on the specific
context. As a means to assess for a multigenerational legacy of diabetes, Scollan-
Koliopoulos, O’Connell, and Walker (2006, 2007) developed a practice-based theory
derived from the family systems illness disability framework and the concept of a
multigenerational legacy of hereditary illness (Rolland, 1994) and the self-regulation
theory and illness representation model (Leventhal, Leventhal, & Cameron, 2001).
