Each employee’s ability and performance in his/ her position should be evaluated objectively, uniformly and with fairness. The assessment shall be evaluated by the employee, the supervisor (reviewer) and/ or the head of department/ division managers. The assessment shall be based on actual performance of the employee during the specified review period. It is therefore critical for all parties to evaluate performance carefully and use their objective judgement. All parties are advised to read the entire form and study the definitions given for each rating carefully, before undertaking the evaluation. Provide supporting comments where applicable and attach additional sheets if required.
