The newborn was extremely unstable and not responding to various
ventilatory modes, and was too ill to be transferred to a tertiary care center. The
addition of oral Sildenafil to the treatment regimen resulted in (1) a significant
increase in oxygen saturation (2) ability to wean the fraction of inspired oxygen
and ventilator settings (3) avoidance of inhaled nitric oxide and other advanced
treatment, stabilisation of the newborn's condition to allow for transfer for further
management. The newborn tolerated the treatment well with little adverse effect
on blood pressure.
