For example, when a patient attended the
clinic, pre-operative supportive care needs were
identified and referrals were made to other
agencies. These included benefit advisers,
dietitians, community macmillan teams and district
nurses. An excel spreadsheet was developed
to enable the CnS to record concerns identified
by patients using the holistic assessment tool.
The data did not identify patients, but allowed
the CnS to gain an overview of the main areas
of concern (Figure 1)
