Nurses did not consistently reassess a child’s pain if the child had a
score of 5 or greater.Of the 27 occa- sions when a painscore of 5 or greater was recorded, a reassessment took place only six (22%) times. The same trend was found in relation to evaluating the effectiveness of pain-relieving interventions.
