k after a couple of days of troubleshooting with BMC support. we figured that in order to find out what is causing the error, we have to turn on SQL logging. this will allow you to see the database tables that it is 'choking' on (i.e. T874, T655, T546) then you can go look in the these tables for the unique identifier fields. filter your table and you should be able to find an entry that is a duplicate.
