Due to the massive amount of skin, muscle, and other tissue that had been debrided from uring K.A.’s abdomen during the course of his care, aggressivewound management procedures were needed to ensure a clean wound bed and enhance the development of granulation tissue. Wound management evolved minute to minute, day to day. K.A.’s nursing team served as the consistent strength assisting with decisions regarding skin management and informing the team of the effectiveness of the chosen treatment regimen. Measures for wound management employed by K.A.’s multidisciplinary team included the following.
- Use and maintenance of the Lahey bag (immediately postoperatively) to decrease insensible fluid losses
- Xeroform dressing over the debrided abdominal wound to maintain a moist wound bed.
- Allograft over the abdominal wound until a graft was available from his own skin cells.
- Autogaft and VAC to assist with closing the massive abdominal wound that had been created as well as a closed dressing to minimize infection risk.
In addition, K.A.’s team alternated the use of the granufoam and white foam sponge when changing out the system every few days. The granufoam sponge has an open pore structure that provides a moreuniform distribution of negative pressure at the wound site. Granufoam is thought to be ideal for chronic, acute and thee sub – acute wounds. The white foam sponge is pre – moistened, less adherent than granufoam, and has higher tensile strength that makes it easier to place and remove.
The development of an enterocutaneuous fistula presented another major challenge for K.A.’s wound management. This is because the enterocutaneouse fistula drained a significant amount of stool onto the wound bed. Effectively managing this problem was a complicated process. Many different maneuvers were recommened by the enteroetomal NP and clinical nurse specialist. In respons to these recommendations, the bedside nurses utilized multiple types of dressing in an attempt to keep the fluid off the abdominal wall. Ultimately K.A.’s primary nurse found that by combining and ostomy appliance over the fistula and the securing the ostomyappliance edges down with the VAC tegaderm we able to collect all the fluid into an ostomy bag thereby keeping the wound bed clean and dry.
Maintaining open discussions and re-evaluation when caring for K.A. was imperative. Whenever there was a change to the plan of care, the nurses photographed the change, including each stage of the dressing change so that all nurse caring for K.A. could relicate what was being done. The photographs protographs proved to be very helpful to the nurse at the bedside.
