To increase the chances of early detection, guidance for healthcare systems drawn up by psychiatrists recommend that the "possibility of a psychotic disorder should be carefully considered" in a young person who is:17
Becoming more socially withdrawn
Performing worse for a sustained period at school or work, or
Becoming more distressed or agitated yet unable to explain why.
Whatever the phase of psychosis presented to doctors, the diagnosis of a psychotic illness such as schizophrenia or bipolar disorder cannot be made with any specific single test.4
There is no biological test for psychosis itself, and when laboratory tests are done, it is because they may rule out other medical problems that provide an alternative explanation.4
Questions for patient and family
Psychosis is primarily diagnosed by clinical examination and history - the doctor examines the patient and asks about their symptoms, experiences, thoughts and daily activities, and they will also ask if there is a family history of psychiatric illness.18
The final diagnosis of a psychotic disorder such as schizophrenia or bipolar disorder is reached once psychosis is confirmed but other causes are ruled out.
Other medical conditions are ruled out first of all, especially delirium (sudden onset of a confused state), butepilepsy and a number of other medical explanations are possible - some of which may be treatable and reversible.4
Doctors will also check for any history of intoxication with drugs, both legal and illegal, and toxins, usually also asking for a urine sample to check this.4
Once psychosis is narrowed down to a psychiatric cause, there are clearly defined criteria that must be met before a diagnosis is confirmed. Two sets of guidance are followed by psychiatrists: the American Psychiatric Association's publication known as the DSM, and the World Health Organization's ICD.
The current publications are APA's DSM-IV-TR - the latest text revision of the fourth edition of the Diagnostic and Statistical Manual of Mental Disorders,19 with a fifth edition in development - and WHO's ICD-10 - the tenth edition of the International Statistical Classification of Diseases and Related Health Problems.5
For a diagnosis of schizophrenia under ICD-10 criteria, psychotic symptoms need to be present for one month or more.16,20 For the complete list of symptoms and how many must be present, see the schizophrenia Clinical Knowledge Summary produced by the UK's National Institute for Health and Care Excellence (NICE).20
Various protocols for the primary care diagnosis of psychosis have been formulated, including one from theIrish College of General Practitioners.21 This guidance helps doctors distinguish types of psychosis.
A schizophrenia and schizophrenia-like diagnosis is indicated by:21
A combination of delusions, hallucinations, thought disorder, disorganized behaviour and negative symptoms present for a month (less if successfully treated)
No mood symptoms, or the mood symptoms are brief in relation to the total illness
Social, occupational or functional decline
No better explanation given by another medical condition or substance use.
For mood (affective) types of psychosis, the guidance lists these criteria:21
A clear history of mania or depression coming before the psychotic symptoms and persisting beyond them
The psychotic symptoms are related to mood - grandiose during elation or nihilistic during depression.
Brain scans
Again, clinical tests do not themselves diagnose a psychotic illness such as schizophrenia or an affective disorder, but tests such as brain scans may be done in the early stages of medical attention so that other conditions - often treatable and reversible - can be ruled out.
EEG (electroencephalography) testing (to record the brain's electrical activity) may be routine and helps to rule out delirium (an acute confused state), head injury or epilepsy as possible causes of psychotic symptoms.4
CT (computed tomography X-rays) or MRI (magnetic resonance imaging) brain scans are not routinely done in the diagnosis of a psychotic psychiatric disorder unless there are neurological signs or reason to suspect another cause for the psychotic symptoms.4 However, it may be useful in ruling out the other causes and in helping to reassure patients that, for example, schizophrenia is the correct diagnosis.4
Treatment and prevention
Antipsychotic drugs
Pharmacological treatment with a class of drugs known as antipsychotics is the mainstay of therapy for people with a psychotic illness.4,22
Antipsychotics are effective at reducing psychosis symptoms in psychiatric disorders such as schizophrenia, but they do not themselves treat or cure underlying psychotic illnesses.4
(If the psychosis is not due to a disorder such as schizophrenia or bipolar disorder, treatment may be different, focused on the cause to treat a non-psychiatric medical condition or injury.)
The World Health Organization (WHO) has a set evidence-based recommendations for antipsychotic medications for psychotic disorders.22
WHO strongly recommends that "haloperidol [branded Haldol in the US] or chlorpromazine [Promapar or Thorazine] should be routinely offered to individuals with psychotic disorders (including schizophrenia)."
Whenever psychotic disorders are treated with antipsychotics, the minimum dose that is effective should be found so that adverse effects are reduced. In women who are pregnant, breastfeeding or planning a pregnancy, "low doses of oral haloperidol or chlorpromazine may be considered."22
Newer antipsychotics are available - known as second-generation - but while their use is widespread in the US, this is controversial4 and WHO does not recommend them, except clozapine (branded Clozaril and FazaClo in the US), which may be used, under special supervision, if there has been no response to adequate dose and duration of the other antipsychotic medicines.22
Acute and maintenance phases of schizophrenia
Antipsychotic treatment of schizophrenia is in two phases - the acute phase to treat initial psychotic episodes, and a lifelong phase of maintenance therapy.4
During the acute phase, hospitalization is often needed. In the maintenance phase, treatment of schizophrenia is in the community and antipsychotics help to prevent further psychotic episodes, although relapses often occur, sometimes due to a failure to take the medications.4
Lifelong treatment of schizophrenia may involve other interventions and support, including the role of family in care.4
