Please be advised that we had a minor first aid incident at a grinder in the foundry. The IP’s finger was lightly abraded (scuffed or sanded) and only hand washing, ointment and bandage were applied. While there was no lost time from work, this could have been much more serious.
The workstation is on STOP work until specific improvements are made:
• Homemade tools eliminated (a makeshift stone dressing “tool” was found as was a homemade hammer. The hammer contributed nothing to this situation, it was just in the area and found during my inspection).
The above bullet, coupled with lack of training and poor 5S practices are the first opinion on cause. Other issues at the scene include:
• Lexan shields to be cleaned and placed in position for proper use (not dirty and set aside)
• Removed guards replaced
• Loose wires out of a plastic conduit removed and replaced with a proper, installation meeting an electrical code (the wires were not connected on either end but contribute to the disarray of the workstation)
• A broken work light, light bulb replaced
• A wooden “shield” to catch grinder exhaust replaced with appropriate collection system or steel shielding
• A training schedule in place to cover those employees that were missed 3 months ago. (This in itself will be dealt with in additional action).

First thing in the morning I want to see the hard copies of the sign in sheets of the grinder training classes that were held 3 months ago, including time of day and shift that they were working on at the time.
