Patients with new-onset, evolving acute kidney injury who require aggressive fluid resuscitation (those with hypotension, low urine output, and rising
creatinine, for example) are best treated in a critical care environment where staffing is sufficient for the intensity of care required. For patients with
end-stage renal disease or terminal comorbid conditions, however, such aggressive care may not be appropriate.
