the unit was implementing the new processes for
prevention and treatment of postpartum hemorrhage.
The unit now debriefs on all hemorrhages
using the performance improvement checklist and
the evidence-based guidelines for obstetric hemorrhage.
The project tracked monthly incidence
rates for the total number of transfusions, massive
transfusions, and unplanned hysterectomies
from January 2009 to June 2011. During this time
period, the unit reduced the incidence of massive
transfusion with five or more units of blood by
33% and unplanned hysterectomies not related to
placenta accreta or percreta by 78%. No readmissions
for postpartum hemorrhage occurred during
this time period.
