HSTRY : Known case of HCC at segment VII,S/P wedge resection for 1 year
TECHNIQUE : Routine protocol of MRI upper abdomen with pre and post gadolinium dynamic scans
FINDINGS : The study reveals mild enlarged left hepatic lobe with a 3.0x3.3x3.0cm at subcapsular region of segment VII/VIII of right hepatic lobe. Adjacent subcapsular retraction with minimal fluid is found. This lesion exhibits central low SI on T1W & high SI on T2W with thick peripheral rim (slightly high SI on T1W,intermediate to low SI on T2W). Post gadolinium study shows no significant arterial enhancement, but washout peripherally in venous and delayed phases. Hepatic and portal veins are patent.IHD and CBD are not dilated. Gallbladder contains no gallstone.
Mild splenomegaly (12.5 cm in length) without focal mass.
Pancreas is homogeneously enhancement, on mass or dilated pancreatic duct.
Bilateral kidneys are normal in size without renal mass or hydronephrosis.
No significant intraabdominal lymphadenopathy.
IMPRESSION :
Mild hepatosplenomegaly
A 3.0 x3.3 x3.0 cm lesion at subcapsular segment VII/VIII of right hepatic lobe as describe above;probably residual resolving hematoma or collection at surgical bed,but tumor recurrence at peripheral cannot be excluded.
